Utah Quarterly Package Program
Ordering Information: The items offered in this program have been approved by the Utah Department of Corrections. Families and Friends of incarcerated individuals may place a Quarterly Package November 1- November 30, 2024. All order privileges and item restrictions are subject to change without notice.
Package & Spending Limit: Multiple packages can be ordered per each incarcerated individual. The combined total of the packages cannot exceed a maximum spending limit of $150 of product.
Eligible Incarcerated Individuals: Not all incarcerated individuals are eligible to receive a quarterly package.
Friends & Family Ordering Instructions: Family and friends may order a Quarterly Package for their loved one by phone at (469) 936-0214 and online at www.UTpackageprogram.com
Incarcerated Individual Ordering Instructions: Incarcerated Individuals may not order for themselves. Only friends and family may order for the incarcerated individual.
Out of Stock Items: If an item is out of stock or unavailable, we will refund the full price of the item.
Catalog Updates: Union Supply Direct attempts to show and describe merchandise and all related information as accurately as possible. However, Union Supply Direct does not warrant that product descriptions or other content of our catalog or website is accurate, complete, reliable, current, or error-free. In addition, manufacturers may make changes in product design, color, package size and style at any time. Union Supply Direct reserves the right to update or correct item information and to change prices and offers at any time. If a product offered by Union Supply Direct is not as described in the catalog or website, your sole remedy is to immediately return the unused item for a refund of the purchase price in accordance with your facility’s rules and regulations.
Payment Method: Union Supply Direct accepts the following forms of payment: credit card or cash payment through ACE Cash Express. Your credit card will be billed upon placement of your order. We accept all major credit and debit cards with the following logos: Visa, MasterCard, and Discover. The following forms of payments are not accepted: Green Dot, Cash App, Apple Pay. There is no surcharge for using your credit card to make purchases. Please be sure to provide your exact card holder name, billing address, CVV code and telephone number that your credit card issuer bank has on file for you.
Sales Tax: Union Supply Direct is required by the State of Utah to collect sales tax on all orders. Sales Tax rates are subject to change at any time.
Shipping: $8.95 shipping fee is charged on all orders (Non-Refundable). Canceled orders prior to shipping will not be charged.
Delivery: Quarterly Packages will be distributed by UCI according to the commissary delivery schedule.
Returns: Incarcerated Individuals will be given an opportunity to inventory the contents of the package in the presence of a staff member. The staff member will verify the discrepancies and report all discrepancies to Union Supply. Discrepancies must be reported at the time of package receipt. Only refunds will be applied for any items found to be defective, damaged, or missing. If the original payment was made by credit or debit card, approved refunds will be refunded back to the card used in the original purchase, including prepaid credit and debit cards.
Customer Service Questions: Utah correctional facilities will not handle any calls or questions regarding the package program. Please direct all calls regarding package orders to the Union Supply Direct Customer Service Department.
